Upon completion, please print, sign and return documents to River City Wrestling at your wrestler's first practice. We are going to have a great season and look forward to seeing you there!

- Medical Waiver-USA Wrestling This form is required before any wrestler will be allowed to practice. Do not fill out the Wrestler's USA Card Number. That will be taken care of at practice.)
